Dear Parent/Guardian: Connecticut state law requires that all students have a complete health assessment during the 6th grade year. The state of Connecticut requires TDap and Meningococcal vaccines be given during 6th grade and these are required to enter 7th grade. If you received a letter in the regular mail from our health office it indicates that your child does not have a health form in the nurse's office that covers these requirements. If you are unsure, please call our health department at (860) 286-2638 to speak to either Nurse Bowman or Nurse Darius. Thank you, Sarah L. Williams
